HEALTH & DENTAL INSURANCE
- Health - CareFirst Blue Cross Blue Shield Preferred Provider Organization (PPO) Medical and MetLife Dental --SNVC Pays 80% of your premium, employee contributes 20%.

FLEXIBLE BENEFITS PLANS
- Section 125 Premium Conversion. The Section 125 Premium Conversion plan allows you to elect to pay your share of the group medical and/or dental insurance premium with pre-tax dollars.
- Dependent Care Flexible Spending Account. Employees may establish an account to be reimbursed with pre-tax dollars for dependent care expenses for children and other qualifying dependents. The account can only be used to reimburse for dependent care expenses that enable an employee (and his/her spouse) to work. The IRS allows a set aside of up to $5,000 per year if married and filing a joint tax return ($2,500 if married and filing separately).
LIFE INSURANCE
- An amount equal to 1 times your annual salary, rounded to the next higher $1,000. SNVC's life insurance plan is provided through MetLife for each employee. SNVC pays 100% of the premium.
SHORT TERM DISABILITY
- 60% of your weekly earnings, up to $1,000 per week. SNVC pays 100% of the premium.
LONG TERM DISABILITY
- 60% of your monthly earnings, up to $6,000 per month. SNVC pays 100% of the premium.
WORKERS COMPENSATION
- In accordance with applicable state law, SNVC pays 100% of the premium.
RETIREMENT PLAN
- 401(K) Qualified Retirement Plan - Tax Deferred contribution where the employer can either match employee's contribution or elect to contribute to the Profit Sharing Plan for the Plan Year.
- Maximum employee contribution is $15,500 (age 21 to 49) or $20,500 (age 50 or above) for 2007.
- SNVC employees, hired on a permanent basis, are eligible to participate in both the salary deferral and the employer matching contribution parts of the plan on the 1 st day of the month following the employee's date of hire, and have an average of 40 hours of service per pay period in the Plan year in order to be eligible for the company matching contribution.
- Company matching contributions vest over a 4 year period in accordance with the following schedule:
- Your 1 st anniversary of employment with SNVC 25% vested
- Your 2 nd anniversary of employment with SNVC 50% vested
- Your 3 rd anniversary of employment with SNVC 75% vested
- Your 4 th anniversary of employment with SNVC 100% vested
METLIFE EMPLOYEE ASSISTANCE PROGRAM (EAP)
All full-time employees will have access to DirectLinc EAP services provided by MHN, Inc. through our disability insurance provider, MetLife. A copy of the DirectLinc EAP brochure is attached for your reference.
The plan is available to you and your dependents and provides confidential telephonic and web based assistance with everyday issues that affect your health, family and work life, such as:
- relationship/marriage problems
- family issues, life changes
- stress/emotional concerns
- alcohol and drug dependency
- financial issues
- child/elder care issues
- pre-retirement planning

* Floating Holidays : Subject to customer requirements and PM approval, you may substitute your birthday, the day after Thanksgiving, the day before Christmas, and/or the day after Christmas for one or more of three floating holidays, provided you do not exceed the total number of holidays in the year.
PERSONAL LEAVE PLAN
- Personal Leave enables you to accrue days off in a single account and use them according to your personal needs. You are able to choose from a bank of personal days to meet you and your family's needs. Your personal leave hours are accrued proportionally on a pay-period by pay-period basis.
- Employed 3 years or less: 120 hours of personal leave;
- Employed 3-8 years: 160 hours of personal leave;
- Employed 8+ years: 200 hours of personal leave
BEREAVEMENT LEAVE
JURY DUTY
- The difference between your salary and jury fee for up to 10 days.
MILITARY DUTY LEAVE
- Leave without pay, continued benefits.
